Eastleigh District has introduced a new form that we would like all members completing any of their Chief Scouts Award to fill in.
The form is to help us avoid the embarassing times when we get either your name and/or your group wrong on both the certificate and the presentation evening.
We ask that you also give us a little bit of information about what you did for the award, so that we can make more of the presentation evening than simply announcing your name.
There are two forms attached (both are the same, just in different formats) Choose the format that suits you best, complete it, and return it to your leader when you have finished your award. Your leader will then pass it on to the District Team, who take care of the rest of the process for you.
